The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an in-person full-time role for an APRN or PA with at least 2 years of experience, ideally in long-term care.
They independently manage patients’ medical needs in PALTC settings, aiming to improve care quality, patient outcomes, and reduce hospitalizations and costs.
Responsibilities include conducting assessments, diagnosing and treating illnesses, developing care plans, providing preventive services, and collaborating with facility staff and families.
They ensure compliance with regulations, maintain accurate documentation, participate in quality initiatives, and promote health and safety.
The AP must work independently, make clinical decisions, communicate effectively, and be proficient with EHR and telemedicine tools.
