What You’ll Do:
- Lead a collaborative care team to improve participant health, independence, and quality of life.
- Provide comprehensive primary and geriatric care tailored to individual needs.
- Partner with nurses, social workers, and therapists to deliver integrated, value-based care.
- Contribute to quality improvement and patient-centered initiatives at the system level.

Ideal Candidate:
- Board-certified/eligible in Family Medicine, Internal Medicine, or Geriatrics.

- Experience in geriatric, primary care, or interdisciplinary medicine preferred.
