Role Overview
Seeking a Physical Therapist to provide personalized, in-home rehabilitation therapy for patients recovering from surgery, illness, or injury. The therapist will conduct assessments, develop treatment plans, and adjust care based on patient progress, working independently within an interdisciplinary care team.
Key Responsibilities
- Perform comprehensive patient assessments and create individualized treatment plans.
- Deliver one-on-one therapy sessions in patients’ homes or care facilities.
- Monitor patient progress and modify plans as needed.
Qualifications & Skills
- Minimum of one year of physical therapy experience or completion of probation as a Rehab Intern.
- Licensure to practice as a Physical Therapist in New York State.
- Valid driver’s license required outside Manhattan.
