Role Overview: This position involves providing outpatient physical therapy services primarily in patients’ homes or independent living facilities, focusing on the older adult population. The Physical Therapist assesses, develops, and implements individualized rehabilitation programs to improve mobility, strength, balance, and overall independence. Responsibilities include patient evaluation, treatment planning, education, and referrals to other disciplines. The role may involve supervising support personnel, maintaining documentation, and ensuring safety standards.
Qualifications & Skills: Candidates must hold a valid state licensure in Physical Therapy, with a Master’s degree or equivalent in Physical Therapy, and have reliable transportation. Experience in outpatient, home, or community-based therapy is preferred. Skills include therapeutic exercises, manual therapy, use of modalities, adaptive device training, and health education. Strong communication, adaptability, and patient-centered care are essential.
