Physical Therapist (Sign-On Bonus)

Position: Licensed Physical Therapist providing in-home, senior living, and virtual care to older adults.

Responsibilities include:

Providing one-on-one physical therapy sessions in private residences and senior communities

Performing evaluations, developing personalized treatment plans, and delivering evidence-based therapy

Supporting patient wellness, mobility, independence, and safety

Documenting care accurately in EMR systems

Collaborating with interdisciplinary teams, including PTs, OTs, SLPs, and Fitness Specialists

Educating patients, caregivers, and staff to promote aging in place

Utilizing telehealth and virtual care to enhance access and flexibility

Qualifications:

• Valid Physical Therapy license or eligibility in practice state
• Degree from an accredited Physical Therapy program
• Proficiency with computers and EMR systems
• Current CPR certification

Physical requirements: Ability to exert medium effort, assist with patient transfers, reposition patients, and perform therapeutic activities; capable of driving to patient locations; able to wear PPE when necessary.
