Qualifications: Bachelor’s Degree in Physical Therapy, licensed Physical Therapist (PT) in Illinois, and Basic Life Support (BLS) certification within 30 days. Must complete continuing education on acute stroke care.
Key Responsibilities:
- Perform patient transfers safely using proper body mechanics.
- Design and adjust individualized care plans based on patient response and evidence.
- Educate patients, families, caregivers, and healthcare team members effectively.
- Evaluate clinical findings, determine diagnoses, and document care accurately.
- Screen patients for therapy needs and supervise support personnel ethically and legally.
- Utilize functional activities and standardized outcomes measures in interventions.
- Collaborate with patients and families throughout rehab, providing education and training.
- Communicate patient progress, discharge plans, and equipment needs with the medical team.
