Role: Hospice LPN Float providing practical nursing care in patients’ homes within the Mt. Vernon area.
Responsibilities:
- Support patients and families by monitoring condition changes and administering medications and treatments.
- Assist with symptom and pain management, ensuring comfort and quality of life.
- Document care accurately and communicate updates effectively to hospice nurses and interdisciplinary teams.
- Help patients and caregivers understand care plans to promote cooperation and understanding.
Qualifications & Skills:

- Strong communication skills for coordinating with healthcare teams and families.
- Ability to monitor and respond to patient needs in a home-care setting.
