We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in patients’ homes, working under the supervision of a Registered Nurse. This role involves delivering and documenting patient care according to care plans and physician orders, assessing patient conditions, and reporting changes. Responsibilities include education of patients and families, maintaining infection control standards, and ensuring compliance with regulations and policies.
- Qualifications: Active, unencumbered LPN/LVN license, current CPR certification, TB test within last 12 months, reliable transportation, and preferred six months of clinical experience. Additional state-specific requirements may apply.
- Skills: Attention to detail, time management, problem-solving, communication, and organization.
- Physical & Environment: Ability to lift 50 pounds, travel, and work in diverse settings, with exposure to infectious materials possible.
