Role: Home Health Physical Therapist
Responsibilities:
- Evaluate patients’ functional status and physical therapy needs in collaboration with physicians.
- Develop and implement individualized treatment plans to improve or maintain patient well-being.
- Perform therapeutic interventions, monitor patient responses, and document progress.
- Educate patients, families, and healthcare team members about treatment plans and goals.
- Complete evaluations, progress notes, and reports on patient outcomes and treatment frequency.
- Ensure care adheres to state laws and standards of practice, working within a multidisciplinary team.
Qualifications & Skills:
- Current Physical Therapy license in the practicing state.
- CPR certification and a valid driver’s license with access to reliable transportation.
- Ability to serve patients in diverse home environments regardless of background or circumstances.
