Role: The Registered Nurse in Home Health provides and supervises nursing care in patients’ homes, ensuring adherence to physician prescriptions and legal standards.
Responsibilities:
- Develops and revises care plans collaboratively with healthcare professionals.
- Supervises home health aides and licensed nurses, providing instruction and task assignment.
- Performs comprehensive assessments, documents visits, and ensures quality patient care.
- Communicates effectively with patients, families, physicians, and team members to coordinate care.
- Participates in team activities, orientation, performance improvement, and on-call rotations.
Qualifications: Current RN licensure, valid driver’s license, vehicle access, and preferred CPR certification. Experience in home health and strong communication skills are advantageous.
