Role: Care Manager (PRN/As Needed)
Responsibilities:
- Coordinate patient care across the continuum to ensure safe transitions and appropriate resource utilization.
- Conduct clinical and psychosocial assessments to develop individualized care plans.
- Collaborate with physicians, nurses, social workers, and community partners to optimize patient outcomes.
- Educate patients and families about care plans and post-acute services.
- Arrange post-acute services and address barriers to care.
- Support complex discharge planning and ensure thorough documentation in the EMR.
Qualifications & Skills:
- Strong clinical assessment and care planning abilities.
- Excellent collaboration and communication skills with multidisciplinary teams and community resources.
- Patient advocacy focus with compassionate, patient-centered approach.
- Proficiency in documentation, particularly in electronic medical records (EMR).
