The Wound Care Nurse Practitioner reports to the Director of Nursing and supervises nurses and medical assistants.
Provides specialized wound care in home, nursing homes, and assisted living settings, including assessments, care plans, patient education, and collaboration with healthcare teams. Requires a Master’s or Doctoral degree in Nursing, current NP licensure, and wound care certification within 6 months. Minimum 2 years of clinical experience as an NP, PA, MD, or DO preferred. Position involves physical activity, field travel, and exposure to biohazards. Valid driver’s license and reliable transportation are essential. Must work independently, make quick judgments, and communicate effectively. Key responsibilities include wound assessment, treatment, supervision of staff, documentation, adherence to safety and HIPAA policies, and coordination with care teams.
