We are seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living facilities.
Our focus is on delivering high-quality care across the spectrum of post-acute settings, from short-term rehab to complex geriatric cases, aiming to improve access, remove barriers, and achieve optimal clinical outcomes.
As a Continuity Physician, you’ll:
- Provide face-to-face primary care services
- Participate in educational series and clinical reviews
- Collaborate with Nurse Practitioners and Physician Assistants
